Health Insurance Claim

Here is how to submit a claim to your 
Health Insurance Company.

1. When ordering your Celebration Ostomy Support Belt request additional copies of your sales receipt.
2. Have your Doctor or Ostomy Nurse write a prescription for your Celebration Ostomy Support belt.
3. Contact your Health Insurance Company and request a claim form.
4. The Celebration Belt is reimbursable through you Medicare supplemental insurance Part A&B coverage.
5. If you are insured by an HMO you may need to contact them to get authorization  before you purchase your Celebration Belt.
6. Complete the health Insurance claim form.
7. Mail the completed claim form along with a receipt for your Celebration Ostomy Support Belt and the prescription from your Doctor or Ostomy Nurse to your Health Insurance Company.
